Dr. Kayla, in our Derby office ONLY, is performing sports physicals for your kiddos. Sports physicals are $25 and you DO NOT need to be patients of the clinic. So refer your friends!!
What you need:
*AN APPOINTMENT. These are appointment only visits and we will not accept walk-ins.
*Paperwork for your particular sport. Paperwork often includes a section regarding immunizations so make sure you have that information from your primary doc’s office.
